GUIDE TO SUBMIT YOUR SUPERBILL
Step 1: Verify Your Insurance Submission Process
Contact your insurance provider to determine how they prefer superbills to be submitted.
Submission methods may include:

You may also ask whether any additional documentation is required for reimbursement.
Step 2: Decide How Often You Would Like to Submit
You may choose to:

Submit each superbill individually after sessions, or
- Submit multiple superbills together at the end of your plan of care
Some insurance companies require separate claim forms for each submission, which may influence which option is best for you.
Step 3: Complete Your Out-of-Network Claim Form
Most insurance providers require an out-of-network claim form to be submitted along with your superbill.
This form:
- Is specific to your insurance company
- Can typically be found on your insurance provider’s website
- May need to be completed for each submission
Step 4: Submit Your Documentation
Once completed, submit:
- Your superbill
- Your out-of-netowrk claim form
- Any additional documentation requested by your insurance provider
